2017 Benefits Open Enrollment

Welcome to the 2017 benefits enrollment period! Enrollment is your opportunity to make changes to benefits, effective January 1, 2017.

This is an active Open Enrollment, ALL benefit eligible employees should waive, review and/or re-elect benefits for the 2017 plan year. Your benefits will NOT rollover from the previous plan year.

Open Enrollment is available from November 15, 2016 through December 5, 2016.
During enrollment you can:

  • Add or opt out of health plan coverage
  • Choose your new plan option
  • Enroll eligible family members in your plans
  • Add, drop or change the level of your insurance coverage
  • Enroll or re-enroll in a flexible spending account to pay for day care or health expenses with pre-tax dollars
  • Enroll in several voluntary benefit options including:
    • Accident Insurance
    • Critical Illness Insurance
    • Identity Theft Coverage
    • Pet Insurance
    • MetLaw Pre-paid Legal

NEW FOR 2017

Aetna Catastrophic NEW!!
The Aetna Catastrophic plan is ideal for those who do not utilize medical coverage often and aren’t concerned about paying for minor health care expenses out-of-pocket as they arise until the deductible is met. This plan has a lower monthly premium and a higher deductible. 

Delta Dental Enhanced NEW!!
Carnegie has made enhancements to the existing Delta Dental PPO plan.  Changes include the addition of adult orthodontia and major care expenses such as crowns, dentures, bridges and implants are now covered at 60%.

Accident Insurance NEW!!
Carnegie offers benefit-eligible employees and fellows the opportunity to enroll in voluntary accident coverage for themselves, spouses and children. The benefits include things like Chiropractic care as well as accident related benefits.

Critical Illness Insurance NEW!!
Benefit eligible employees and fellows have the opportunity to enroll in critical illness coverage for themselves, spouses and children. This benefit provides additional compensation based on condition/diagnosis. This benefit is available with no underwriting.

Lifelock Identity Theft Coverage NEW!!
Carnegie offers benefit-eligible employees and fellow the opportunity to enroll in Identity Theft Protection services through LifeLock. There are three levels of coverage available including coverage for children under 18. 

Enrollment: Complete the enrollment form and send it to Human Resources at hr@carnegiescience.edu

Rates: are based on plan selection. Send the completed enrollment for to Human Resources. The form will be processed and premiums will be deducted semi-monthly.

MetLaw Pre-paid Legal NEW!!
Carnegie Science offers benefit-eligible employees the opportunity to enroll in voluntary pre-paid legal services through Hyatt Legal.  This plan covers spouses and children as well. And provides legal assistance in areas such as: Estate planning, Civil lawsuits, Money matters (including taxes), Real estate matters, etc.

Pet Insurance NEW!!
Carnegie offers benefit-eligible employees and fellows the opportunity to enroll in Pet Insurance Coverage through VPI or Healthy Paws. VPI has three levels of coverage available including wellness care for your cat or dog.  Healthy Paws has individual quotes available depending on the type of pet you have. 

Enrollment: visit www.PetInsurance.com to enroll online or call 877-738-7874.

Rates are determined by the age of your animal and the amount of coverage. Once rates have been determined by VPI and Healthy Paws they will sent the rates to Carnegie payroll and they will be deducted semi-monthly.

Please review your options and costs carefully. You can make any changes you like at home or at work until the end of the enrollment period. Once the enrollment period has ended your choices will be final until the next enrollment period or until you have a qualifying life event. Contact your Human Resources if you have questions.

ENROLLMENT

Open Enrollment is accessible through ADP Workforce Now

1. Log on to the Employee Self Service Web site https://workforcenow.adp.com
Click here for - ADP Employee Self Service Registration Guide with Registration Code (It is REQUIRED to be logged into your Carnegiescience.edu Google Email to access this file)
2. Complete all required fields.
3. Click OK - Welcome screen is displayed. A data update wizard will bring you through the process of confirming and updating information on yourself, your dependents, and beneficiaries.
4. Navigate to the Benefits Tab and select Review/Change Benefits.
5. *Recommended* ‘Walk me through this process” option to begin enrollment process.
