The 1095-C contains detailed information about the healthcare coverage provided by The Carnegie Institution for Science.

Who receives a Form 1095-C?

All employees or any individual covered under Carnegie health insurance should receive a 1095-C form.

When will I get my Form 1095-C?

You should receive your 1095-C for the 2019 tax year before by February 5, 2019.

Do I need my Form 1095-C and my W-2 to file my taxes?

You do not need your 1095-C to complete your 2019 tax return. The IRS Q&A clarifies that taxpayers do not need to wait until they receive a 1095-C form before they file their 2019 taxes. Taxpayers comply with minimum essential coverage filing requirements by simply checking a box on your 1040 that you had coverage for the year - you do not need to file your 1095-C with your tax return.

Under what circumstances would I need the information on Form 1095-C to file my U.S. Individual Income Tax Return?

Only if:

  • You received an advance premium tax credit or wish to take the premium tax credit on your return; or
  • You had a gap in coverage of more than three consecutive months.

How will the Form 1095-C impact my taxes?

If you do not have health care coverage and do not qualify for an exemption, you may be subject to a fine when you file for your 2019 tax return.
